Your CFI and the Examiner should be using the latest version of Form SRG\1105
http://www.caa.co.uk/docs/33/SRG1105.pdf which is a combined PPL(Aeroplanes) and FRTOL application form. It's only been out since September 2007, after all.......

The fee for the initial grant of a PPL(A) with FRTOL is £168. For re-issue (thanks to the JAA...
) after 5 years, the fee is a further £67. All due for review in March.
