I've had screaming babies in FIRST on BA. Seriously, what can you (legally) do? Name me an airline that bans children in their premium cabins and I will fly them.
As for QR crew, put yourself in their place, where you can be fired for the slightest infraction. You go up to a family travelling in a premium cabin and tell them to be quiet If this family is travelling in a premium cabin, it is likely that they are very wealthy and, importantly, connected. Qatar is a small country. All this father has to do is complain to AAB and next thing, this poor crew member, who is trying to help you out, is fired. After being fired by QR, this crew member can't even go home until they have paid their bond back. Of course, they are evicted from QR accommodation, which means they are stranded and homeless in Qatar.
As for your ground staff comment, I couldn't agree more. BA have outsourced ground handling in a lot of airports and I've seen the results of this in the complaint statistics.
