I only posted the NTSB report as the crash was mentioned in Part 1 of the article linked to by HOSS 1 at the top of the thread, and as several comments on the web alluded to the fact that it was due to Georges Van Nevel being a cowboy (in fact some comments on the web are blatant in their name calling).
