My SOPs strictly separate the two, it's either all automatic or all manual. No manual flight / autothrust on, not even on the A320.
As far as I know, MD-11 (and I believe also DC-10) SOPs allowed - even encouraged - "manual flight" with auto throttle engaged. On the B737 this is strongly discouraged, except during takeoff and initial climb.
