I too witnessed one of said incidents, can't help but wonder if we didn't view the same one from slightly different angles. Part of the problem is when the instruction immediate is used but not executed. A case of "I've got the runway" perhaps?
Real Slim, agreed, always works best when everyone is well briefed and situational awareness being fully utilised.
