If we disscus about safety,safety measures,SMS and similar things I have better example,
Everyday they put money before safety of the flying public and its not going to change.
Near air miss- 20 % salary reduction for next 6 months,

Coordiantion mistake- 20% salary reduction for next 3 months

Wrong instruction(with no fatalities)-20% for current month

Different opinion (non ATC related)-out of any promotion until next political election

List is endless,but my favourite part is : management is aware that mistake is never alone(somethimng wrong must happened before) . So you will "catch" more than one "measure" per case.
On example :
If near air miss happens due to coordiantion mistake ,followed by wrong instruction than "measure" will be:
60 % salary reduction for next 10 months. Promotion is stopped as well. Because you aren't safe. No political reasons.
The only positive thing in such example is: I am out of such generic safety organization.
I can't help to your management but I could try to be positive.
