1. DCA inspectors notified Orient Thai management of the excessive working hours of the pilots after the accident of (and presumably related to) OG269
2. Orient Thai management acknowledged the truth of excessive hours at that time and continued to fly hours in excess of legal maximums.

4. "Flight operation officers did not obtain fudamental training and procedures in case of emergency"
5. "Flight operations officers of OTG [One-Two-Go] had to work with those of OT [Orient Thai], meaning their duties were inseparable"
