DGCA ATPL conversion exam question

Hi guys,

I am in the midst of filling out the ATPL conversion exams to get the Indian ATPL and found out that they have revised the format.

For the ATPL conversion exams , is it required to fill out the Annexure 1( it kind of resembles a FAA endorsement to take a test)
I have a FAA ATP , who can sign this document??

Any advice/help will be greatly appreciated.
