The definitive list is maintained by ICAO in document 8585. Can't find a link on the ICAO site but the Danes have this one ICAO DOC 8585
Yes, ICAO believe they are above having to provide any public face, and thus their documents are available only on a paid, subscription, basis. Some States which have 'freedom of information' legislation, and take it seriously, publish for them.
Utterly pathetic.
ICAO, shame on you.
