Are you/eligable for 'sick pay'? If so see an MD/doctor. Tell the doc you have a very bad back, searing pain when you cough above the spine/pelvis area, painful 'twinge' (lower back) when leaning forward making walking painful.
Thing is with back pain the doc cannot disprove your symptons. Go for it, play the game to your advantage.
