Pilot error in reaction to the flap lockout I don't have any complaints on because that's where all this seems to point to. There seems to be a lot of parallels to the Garuda accident. Sorry, you picked a better word, pilot error
precipitated by flap lock-out
And that to me is a lot more believable than
The approach was rushed, to the extent that normal landing flap and Vref(30)+5 was not achieved.
