Flight Crew Licence Check?

While looking at the ATSB website I found this;
Flight Crew are often required by prospective employers to obtain a flight safety record statement from the Australian Transport Safety Bureau (ATSB). To accede to the request, the ATSB conducts a search of its air safety occurrence database which, contains record of air safety occurrences from 1969. The result of the search based on information recorded is then provided to the applicant on ATSB letterhead. However, it must be noted, that the information on the database is not stored for the purpose of maintaining records on an individuals involvement in an air safety occurrence. Consequently, the ATSB cannot warrant that an individuals involvement in an air safety occurrence could be accurately discerned in every circumstance from the information recorded. The information contained in the statement is supplied solely at the request of and in accordance with the applicants rights under the Privacy Act 1988.
To facilitate such request, the ATSB has designed a Flight Crew Licence Check Request form that must be completed and forwarded to the Information and Coordination Section of the ATSB for appropriate action. For privacy reasons, the form must be signed by the applicant.
