SOP for us-

If in cruise or if time permits, call the FA's forward to help remove the Capt from the seat, get him into the cabin for medical help (EMK/Doc/Etc) and to reduce the chance of a partially mobile flailing incapacitated capt. being at the controls if they regain consoiusness later in a reduced level of awareness. Then take the right seat again (familiarity and all) and excecute the emergency, and stop on the runway. Set the brake, get up and move into the left seat - taxi with the tiller to the gate asap for normal medical personnel to get the incapacitated captain medical help, unless a remote portable stairway exists for fast ingress/egress of medical help. Use resources, inform people who can help asap and let the emergency procedures and protocol work for you.

Yes we train FO's how to use the tiller. It's really a lot less difficult to taxi than to fly an airplane.

If you are low to the ground and can manage a normal landing with an incapacitated person in the other seat (not flailing, kicking or not slumped over) then try at least to move their seat back, have their shoulder harnesses set to non slip (keep them out of the controls) declare a medical emergency and have the ambulence ready, land, use diff breaking or limited nose wheel authority to make a high speed exit (at a low speed of course) and set the brake. Get the incapacitated person out of the tiller seat if you must, so you can sit in the left seat to continue via the tiller to a remote stair or a gate for help.
