It usually means that they start from different locations. For instance, one may be from the hold over the field, followed by a procedural outbound leg, procedure turn to join the ILS. Another may come from a point on a star direct to the platform at a certain distance out on the localiser. Hence they have a different designator. As TopBunk says though, they use the same ILS facility ultimately.
