Poor Cockpit procedures

On the daily NAT track message, the following remark is always there.

"80 PERCENT OF GROSS NAVIGATIONAL ERRORS RESULT FROM POOR COCKPIT
PROCEDURES. ALWAYS CARRY OUT PROPER WAYPOINT CHECKS".

What are your current procedures?, What did you do to prevent errors?, where can you read more about these errors?, what are the reasons for the poor procedures?
