I am doing a bit of research accidents and incidents that had causal factors associated with maintenance failures, particularly the human factors aspect.
Causal factors such as errors induced by organisational failures, fatigue related issues, shift handover, time pressures etc.
I am looking for accident or incident reports that specify or allude to some of these issues. Any suggestions or links would be much appreciated
