I work on obtaining o/f and landing permits for an American B757/767 operator. We do a lot of charter - holiday type - flying and a lot of flying on behalf of the military.
One of the best resources is the Jeppesen "Entry Requirements" directory. Check out their site for a web-based subscritption. It lists the fax numbers / AFTN/SITA addresses to send requests to and just what to expect and what is expected of you as an operator.
