If I understand your question... you don't have much choice here..
As far as I know there are four medical centers which can assign Class 1 certificate. Two of them are in Warsaw (WIML and LIM), one in Krakow (LIM) and one in Wroclaw (GOBL). Personally, I recommend WIML (Polish military facility) - you can make it really quick there!
