As a panel operator on the old B-707 sim I once gave as an input a N1 governour failure (that means that the engine accelerates well above TO thrust) imediatly after the thrust reverser actuation on an outboard engine on a slippery runway. On the 707 u can transform 50% of the thrust in reverse thrust. The upgrading captain didn't shut off immediatly the outboard engine at full reverse thrust. Can somebody immagine what happened. The plane, i mean the simulator, hit the mud.
