Is there any way to change a composite detail, once it has been moulded into a final shape and cooked hard?

Metal aircraft are often changed significantly. People commonly rip huge holes through passenger aircraft fuselage, put a cargo door and doorframe reinforcement in the hole and expect the thing to fly and hold pressure. Someone is actually now working on second-hand Boeing 747-s, sawing off the whole upper fuselage behind upper deck as well as the whole tail, replacing the upper fuselage with something much bigger than a Airbus 380, fixing the tail to the rest by hinges and locks - and they expect the thing to fly!

Could you do the same to a Boeing 787?
