What a shambles! Has this firm never had a Chief Pilot. Or just lickspittle taking the position for perceived w@nk value and not enough nous or industry experience to have had serious enough concerns say six, twelve or even 24 months ago to see where the deficiencies (as described above) in the Operations Manual were leading.
