conclusion of the report was that the controller was at fault in this incident, the other layers you have mentioned have no bearing on that.
If the Ryan had been taxiing more slowly it
might not have happened, if the Airbus had had his lights on it
might not have happened. Had the controller not got it wrong it would
not have happened, the Ryan having confirmed his clearance to DZ1 short of 06L.
From your last comment i take it you stop at a green light to make sure it's safe prior to proceding, making you a hazard to road safety
