Exclamation FAA HKCAD DCA607 Conversion

Hey all,

I am an FAA-licensed pilot picked up by an airline and converting my license to a Hong Kong license. In their license conversion form, they ask for "Details of the most recent Instrument Rating flight test on both Precision and Non-precision Instrument Approaches in a Multi-engined Civil Registered Aircraft or Simulator".

Does this mean the approaches had to be done during my instrument rating? Or could this be as simple as an instrument proficiency check done with an instructor? During my checkride for my initial instrument rating, I only executed a single non-precision approach. The supporting document the company is asking for is the FAA Form 8410-1, but to my understanding, this isn't accessible to the public, and there is very little information online on how to go about filling it out. Could I use another form for HKCAD, such as a written report from an instructor or another form?

On the DCA 607 it only asks about a precision and non-precision approach, so maybe HKCAD do not require the 8410 form my company is asking for perfectly filled out?
