IF you using Radial In function after setting FDs off then In that case the FMS will not sequence the new-created TO waypoint when the aircraft overflies it. It’s an abnormal behaviour you can read about in the FCOM—> AC Systems-Auto Flight-Flight Management - FMS2 Thales Temporary Abnormal Behaviours-Waypoint not Sequenced. It was driving me crazy before I knew about this glitch as many times I ended up on final with the TO waypoint behind me and I could swear I had sequenced it! In that case you just sequence manually the first waypoint or use Radial In before you set FDs off. If you want the from waypoint simply reinsert the approach into the FMS. But why would you want it tho? Personally I just clear waypoints sometimes until the threshold if I have to. Less workload.
