As NAB said, instead of stating the obvious, and something that has been raised many times within MoD itself, perhaps the committee should have interviewed (a) someone experienced in successfully 'addressing the crisis', and (b) someone who had rejected the proposal to learn from experience. There are many of the former to choose from. Before I retired the latter was a named 3-Star, the Deputy Chief Executive.
The official explanation, once given to me by a 2-Star, is this is seen as raising the bar, which is simply not tolerated as it raises expectation and highlights torpor.
