one just has to look at the extent to which our military brethren have reduced their mishap rates in an hazardous environment
All Oz military flights in my day required signing off by an authorising officer, that is someone who was cognizant of all the details of the flight to be undertaken and the competence of the crew, if the facts didn't mesh, no authorisation, could be lack of crew experience, weather, or any number of things. Even with all that they still have accidents. Even there the authorisation system can come unstuck, lad from Sale died along with his navigator in an F-111 when the authorising system was less than optimum. The leaders in the safety stakes are probably the airlines, but we see close calls even there in Oz airlines.

A lad on his first job starting work at the beginning of the wet, even with the most liberal and caring of operators, is on a very, very steep learning curve. We've all had that learning experience which we were fortunate to survive. RIP young man and pax.
