Why? Personally I put it down to education that is not based on challenging authority. Processes are devised that generate work and preserve jobs instead of removing manual labour and increasing efficiency.
If you're too efficient you might lose your job. I believe that's called "Poverty Culture" and it's prevalent in the back-office.
An IT Manager told me one of his staff had instructed a co-worker "Your job is to keep your Manager happy"
Says it all really.
