K-19. Both cases, public word was that the boats were damaged around the bow, and for the Gato, she got some damage around the "reactor plating area". They don't fit the damage of your picture, but that sure looks like US of A periscope camo, and the configuration looks very much like a Permit boat, needing some bondo fill.
