Several mistakes were made by the controllers that contributed to the PSA 182 collision: ATC did not restrict PSA 182 to 4000 ft over Montgomery as required, ATC radar seperation was not used when it was available, ATC conflict alert was not conveyed to pilots, ATC were not proactive to evident loss of visual on traffic by PSA 182.
