When those passengers require additional specialist attention, such as on casevac flights, then supernumerary crew (in this case probably RAF Medical Branch and PMRAFNS personnel) are added to that basic crew. When passengers are carried, as they require extra specialist attention, supernumerary crew (in this case RAF air and ground steward) are added to that basic crew. You still need the core basic crew defined by the MAA as Aircrew, which as pointed out above, does not include CC.
