To correct some details, it was a B3 (G-HKCN) that crashed after a fuel bowser cover (not tied down - it was considered snug enough that tie downs weren't necessary) was recirculated in the downwash and went through the blades. Occurred off the coast of Norway, 2017. Here's the report
