update - apparently there were several uses and they worked every time.
I can think of 5 ejections from the T1:
Inverted spin in Scotland - 2 pob, double ejection
Prop fatigue fail near Driffield - 2 pob, double ejection
Not understanding gate heights - 1 pob, single ejection.
