Scathing ACCIDENT Report,indeed.Unique insight with the whole sequence being visually recorded from the jump seat. PNG/PX now has no effective Pilots Union to oppose the"proposed" introduction of Flight Deck Video cameras,so ,likely it will happen.Sure makes the Accident Investigators job much easier! PX MD/CEO/Boss Man, (or whatever his title is,) is still calling this an INCIDENT, & Human Factors ??? are to blame (ie NOT Pilot Error) (in todays Pos Guria).He may need to brush up on the Definition of Accident vs Incident.Sad outcome for a once Great Little Airline.
