Looking for crash info, long time ago

In a discussion about (elevator) stops in Cessnas I refer often to a sickening crash that happened 35-40 (?) years ago.
The twin Cessna flew single piloted to a location (Bermuda? Not sure) to pick up a full load of pax.
He arrived OK, loaded and departed.
The plane was seen to pitch up after T/O and crash, killing all aboard.

Investigation showed the unsecured Elev. down stop had worked itself loose during the positioning flight. Because of the CG shift backwards now nose down trim or elev. input was needed.
But the elev surface was blocked to do so by the limiting screw.

Does anyone remember that crash so I can look it up? I cannot remember the exact type.
