Only very experienced pilots, the Bob Hoover's, de Crespigniny's (with help) and Chuck Yeager's, who are thoroughly familiar with the aircraft that they are flying and with extensive non-auto training can deal with these highly abnormal situations
Two of those esteemed gentlemen had there days. Hoover refuelled his piston Aero Commander with jet fuel, double engine failure on take off and written off airframe, also hit the runway during a display in the Aero Commander grinding off the belly skin. Yeager had to throw away a F-104, nearly losing his life in the process, when his instrument flying skills were not up to the task, ego wanted another altitude record so used authority to usurp the project pilot.
But life being what it is, every year or so we must accept that such events will occur and that we cannot reduce this to zero.
Forget whether it was IATA or ICAO releasing safety figures some years back. They had a zero for some particular statistic, the media asked whether they would be breaking out the champagne to celebrate reaching the holy grail. The reply was, "No, we regard the figure as a statistical aberration that we don't expect to ever see repeated".
