The FAA allows a foreign medical to be used when you excercise the privileges of your 61.75 piggy back certificate.
FAA medical or medical of the country or original license issuance.
As far as the FO, to excercise the privileges of a a FAA Commercial certificate he needs a Second class medical. His First class will downgrade to a Second class 6 months or 12 months after initial issuance depending on his age.
