Iran Air 655 A300 (neither found)
El Al 1862 B747 (CVR not found)
American 11 B767 (obvious reasons not found)
United 175 B767 (obvious reasons not found)
Asiana 991 B747 (neither found)
Malaysia 370 B777 (neither found)
So, seems a few missing CVR/FDR from wide bodies in the last three decades and quite a lot more from narrow body jets.
However, with the exception of MH 370, the cause(s) of the crashes were determined.
