Locked door: Oh dear! Reference to the full report does in fact show that BA came in for criticism for their use of a non - Boeing standard fuel balancing procedure. This, coupled with the crew's apparent poor/incomplete understanding of the fuel system turned what should have been a routine 3-eng fly on into an emergency!
