As far as I remember AJX required a licence verification from the issuing authority and had a field for any "accidents or serious incidents" on the application form (through PARC).
My authority (ZAF) issued a "Licence Verification" document which basically stated the licence I held and that I did not have any
current or pending investigations. This sufficed for AJX when I interviewed with them back in 2016.
