People make mistakes, all of you have, I certainly have, owning up to them and passing on the lessons learnt stops them from happening again, with potentially much more serious consequences. If it was indeed a technical or mechanical fault, then I'd appreciate knowing what and why so I can formulate my own plan of action on how I'd handle something similar and add that to my tool box.
ATSB publishes any safety issues as they are encountered during the investigation.
