If it only had "AFE" on it in light grey letters in the bottom right corner, it would look exactly like the ones I have in my flightbag.
Personally, I used some Jeppesen ones in the US last year - I thought they were pretty good, and when folded correctly after completion only the most relevant stuff was left on display on the kneeboard.

Well here is my version. Mine will also calculate your heading and windspeed from true heading and true airspeed ;)

Those are some truly scary calculations, did you work them out yourself or get them from a book or what?

I worked out the formula's myself. If you look closely enough, you will see that they are repeated within each cell a fair bit. This was to get around calculations sometimes working out at for example 375 degrees instead of 15 degrees etc.
It's not as complicated as the formula's look :)
But it works quite well.
