How do airlines decide how to design the airline-specific engine-out navigation procedures for the different airports they fly to? Does ATC have to be aware of where each aircraft plans to fly if it has an engine failure?

Thanks,

Sam

Some airlines do this in house, others use a contract runway/obstacle analysis chart service.

As for ATC, yes many times they have a copy, however, in the engine inop climb case, the pilots must announce that the EO procedure is to be followed.
