Which airlines have an In Flight Entertainment System (seat back) display of feed from an exterior view camera, and which have audio feed of ATC?

United have audio feed on channel 9.
bmi have an exterior camera, don't know about anyone else though.
SK have exterior cams (front and underneath) on their A343s.

CX has undernose cams on their 777-300s (but they don't seem to be on very often these days).

Finnair Airbusses have exterior cameras, forward facing on take-off & landing, downwards on approach & departure.

Air France on thier A330's
My Travel A330's
Emirates A330's
All 3 have landscape cameras
Emirates 777s have them, on take off and landing the nose camera view is also shown on the main screen.
I was fortunate to experience a landing at Kai Tak with this entertainment.
On recent flight Malaga to Munich - the Condor 757 had exterior cameras (forward facing) for departure and landing .

Thai have a forward looking camera on their 777's.
TG also have them on new config 744 and 345 & 6 (Ch 97)

ANA has exterior cameras. Why don't more airlines (e.g., US-based) have them? Is it that big a deal to install and maintain? They are entertaining.
