When is aircaft classed as 'heavy'?

On reading through these forums i quite often see the term 'heavy' to describe aircraft? When is an aircraft termed as 'heavy'?

It used to be a Max Take-off Mass of 3000,000lbs but I believe it is now 255,000lbs.

Heavy Vortex Wake Category = MTOW 136,000 Kg or greater. See:

:8 - whoops!

I did see, however, a reference to something American saying it was now 255,000. Was that implemented anywhere?
