Fatigue is one factor, MACHO factor is another reason.
I can do it, I can't be wrong, It's too embarrasing, those attitudes are a sure recipe for disaster.....
And the Capt was not the only one with those attitudes, the entire organisation is full of people with that mindset, that includes Pilots, trainers/examiners and the management.
A tragedy could have been avoided.........
Atleast, the current & next generation of pilots need to learn a thing or two from this accident......

The final DGCA enquiry report on this incident has been available in the DGCA website for public access for quite some time.
Look under "Aircraft" then "accidents and incidents" and then "reports". The AIX one is filed under "incidents" You will find quite a few other interesting ones as well.
