My biggest bug bear is installations failing to have big bold names displayed at appropriate positions. Globally this is the case and the deck shouldn't get certified unless the operator does so....

Agree with Crab on this entirely. Totally avoidable tragedy, very sad. Hope the report doesn't gloss over basics, because lessons need to be learnt. But not the hard way like this one.
