I've just seen some new pictures on BBC and engine 4 clearly has the thrust reverse doors open.

You forget that the B7E7 is Boeing's response to the A332. Although the B7E7 will be more efficient than the 332 it is only just a bit different.

All the 20% fuel savings etc...

Winglets/fences and raked wings are more popular now especially when you need the performance produced from a larger wing area but don't want to increase the wingspan...
