Can anyone explain why the incident happened when it did, just before the runway? Why did it not happen earlier in the flight? To me, this is just too much of a bit of luck to have been pure...

Isn't there a fuel cutoff switch in the B777 cockpit? Is it clear beyond any doubt that the crew didn't hit the switch by mistake? Would such an event be something that the "black box" would...

Cavitation on other Aircraft of this type?

That cavitation was present is interesting, but not dispositive, unless it is so unlikely to be found on other aircraft of this configuration that it almost certainly points to icing as a cause....
