I have no clue what caused this accident at this time, but perhaps it was low cabin pressure / hypoxia. A second pilot may help in some situations or cross checks, but if there is not enough oxygen...

Would be interesting to see what the default behavior of the C560 FMS is after completing its route without landing and remaining on autopilot. Perhaps it was simply heading to the departure...
