The expenses form that they email you is just called "Expenses Form" - with no file extension.. if you rename it to "Expenses Form.pdf" it should open fine in acrobat.

I was worried about asthma too but I also got through with no problems. As long as your peak flow is OK and you have it under control I'm pretty sure you'd be fine. :)
